Medical Certificate Request Form

If you're unable to attend work, school or university because of illness, you can request a same-day medical certificate online through Medi Certificate Online. Your request is assessed by an Australian registered medical practitioner and, when clinically appropriate, an approved certificate is emailed directly to you.
